Never events are medical errors that are “entirely preventable”, according to NHS Improvement, because guidance or safety recommendations are available nationwide. It was recently reported that two never events had taken place at Sheffield Teaching Hospitals.
Documents published by Sheffield Teaching Hospital NHS Foundation Trust showed that a woman lost an ovary due to an error whilst undergoing surgery to have her womb removed as part of her treatment for cervical cancer. During the surgery, the blood supply to the woman’s right ovary was mistakenly tied up causing the tissue to die. As a result, her right ovary and fallopian tube had to be removed.
In another incident, a patient was given an appointment in oral surgery to have a biopsy on the right side of their mouth, but when they attended the appointment, the biopsy was taken from the left side in error.
Dr David Throssell, medical director at Sheffield Teaching Hospital NHS Foundation Trust, has apologised for the two errors made.
