An investigation into Morecambe Bay NHS Foundation Trust has found that the deaths of 11 babies and one mother at the hospital were the result of major failures in care.

BBC News reports that the report into the scandal found standards of care at Furness General Hospital in Cumbria amounted to a “lethal mix of failures”.

Health Secretary Jeremy Hunt told the Commons that the Morecambe Bay NHS scandal was a smaller scale version of events that took place at Mid-Staffordshire NHS Foundation Trust.

The inquiry into Furness General Hospital founder the maternity unit had been dysfunctional and had provided poor standards of care to babies and mothers – and staff on the unit had “deficient skills and knowledge”.

The inquiry also found that midwives on the unit referred to themselves as the “musketeers” – and working relationships between midwives and doctors were “extremely poor”.

Monitoring and regulation of the hospital had also been poor – and the report said there had been a “significant organisational failure" on the part of the Care Quality Commission, with reports on the hospital from healthcare inspectors being “misleadingly optimistic”.

The North West Health Authority and Parliamentary and Health Services Ombudsman were also found to have failed in taking opportunities to flag up problems sooner – problems which began in 2004 were not addressed until 2011, despite a cluster of incidents in 2008.

The inquiry has made 44 recommendations, including staff involved in caring for patients at the unit between 2004 and 2011 being investigated by General Medical Council and Nursing and Midwifery Council.
