£20,000 fine for contractor after worker falls down shaft at De Montfort University (4 April 2016)

The Health and Safety Executive (HSE) has prosecuted a Derbyshire-based company, after a worker from Romania was injured after falling down a service riser shaft.
An investigation into the incident by HSE found that, on 15 June 2015, the unnamed 36-year-old worker was dismantling falsework – a form of temporary structure – in a building that was under construction at the Fletcher Development, at De Montfort University in Leicester.
The worker fell 13 metres down the service riser shaft – and the Health and Safety Executive investigation into the incident found that the contractor, David Ashley Construction Limited. was guilty of providing an unsafe system of work, as well as inadequate supervision of workers.
At Leicester Magistrates’ Court on 31 March 2016, David Ashley Construction Limited of Lydford Road in Alfreton, Derbyshire pleaded guilty to breaching Regulation 4(1) of the Work at Height Regulations 2005.
The company was fined £20,000 and ordered to pay costs of £1,776.
